Vermont Nurses In Partnership is conducting research related to the transition to practice for new graduate, re-entry, and new to specialty nurses.

These survey tools are to be used in conjunction with this research only.

Please do not submit data on one of these forms unless you have been instructed to do so by your educator or site coordinator.

NOTE - all data is encrypted during transmission. All information that is collected via this form will be held in strict confidentiality. It will be shared only in aggregate format without identifying data.
